What is Living Well After Stroke?
Living Well After Stroke is a free group-based health promotion programme which aims to provide people with a ‘toolkit’ of skills and strategies for making positive, healthy changes that can reduce the chance of another stroke.
This programme addresses healthy eating, physical activity, fatigue management, alcohol intake, smoking and adherence to medication. You will be given information and signposted to other services that will help you achieve your own personal goals and maintain a healthy lifestyle.
The Living Well After Stroke programme is delivered over an 8-week period (online or in-person). You will have an initial appointment with a CSS nurse who will discuss the programme with you and provide you with the living well after Stroke workbook.
This will be followed by four group (1 hour) sessions facilitated by a health professional coach. The small group dynamic (4 to 6 participants) will motivate you and provide encouragement and support. You will be asked to fill out a questionnaire to evaluate the programme.
Who is Eligible to Participate
You are eligible to participate if you are
- Aged 18-years and over
- Have had a stroke or TIA between 3-months and 5-years ago
A spouse or partner may accompany you, if it would be helpful to achieve your goal.

The Programme has been adapted for use in an Irish context by Cork Stroke Support with
the permission of LWAS@strokefoundation.org.au
